In the need to protect our staff and patients, White House Clinics is requiring COVID-19 vaccination for all new hires prior to starting employment.


  1. Participate as an active member in the patient’s outpatient primary care team. This includes, but is not limited to:
    1. Maintaining availability and accessibility to other provider staff for interventions related to patients’ behavioral health needs;
    2. Assisting colleagues in managing behavioral health crises in the clinic;
    3. Maintaining a visible presence in the clinic when not engaged in therapy sessions to promote collaboration between disciplines; and
    4. Using shared EHR and care team huddles to provide insight and direction related to behavioral health needs.
  1. Utilize clinical interventions from a variety of theoretical bases to treat patient mental health or substance use conditions, including enhancement of motivation to address mental health, physical health or substance use conditions, development of diagnosis, treatment plan, and plan for termination of treatment episode.
  1. Assign appropriate diagnosis(es) for a range of behavioral health conditions, including substance use disorders.
  1. Engage patient in identification of specific treatment goals and interventions/supports needed to achieve these goals. Utilizes standardized screenings and assessments in accordance with organizational clinical practice guidelines and quality improvement initiatives.
  1. When applicable and as indicated by patient need, provide individual, family, couples and/or group therapy to patients.
  1. Provide care management services to patients with behavioral health and substance use needs, including follow up on referrals to appropriate outside or specialty behavioral health resources and collaboration with Enabling Services to ensure that patient psychosocial needs are addressed.
  1. Maintain problem-oriented, timely clinical records.
    1. The records are adequately detailed, truthful, clear and accurate to allow good continuity of patient care and accurate billing (coding of diagnoses, services provided, and charges).
    2. The Behavioral Health Provider’s clinical notations should facilitate another provider’s assuming follow-up care. Notations should be completed on the day that services are provided.
    3. The documentation should reflect medical necessity of service, interventions utilized, progress in treatment or change in treatment plan if patient is not progressing.
  1. As requested, participate with MSW programs in student practicum rotations.
  1. Participate in Health Help staff meetings and quarterly provider meetings as requested. Quarterly meetings will occur outside of normal work hours.
  1. Assist with other duties or in other departments as instructed by supervisor.


Minimum Education
All Behavioral Health Providers will have graduated with a Master of Social Work degree from an accredited institution of higher learning, and passed the ASWB Master’s Level exam.
Minimum Work Experience
Two years experience in a patient care setting.
Licensure Required
Maintains unrestricted Kentucky CSW license and meets ongoing licensure requirements.
CSW’s in the Behavioral Health Provider role must have received the Certified Social Worker license from the Kentucky Board of Social Work and practice under a Board of Social Work approved Supervision Contract, pursuant to 201 KAR 23:070.
Supervision of Certified Social Workers
White House Clinics will provide a minimum of 2 hours of clinical supervision every 2 weeks in compliance with 201 KAR 23:070, from the start date of the supervision contract until no more than 6 months after the CSW has completed the 200 hours of required supervision. If the CSW has not received the LCSW license by that date, the CSW will be responsible for securing the required ongoing supervision. CSW’s are responsible for recording dates, times and content of supervision sessions and adhering to the direction of the Supervisor of Record.


Successful applicants will be self-directed, mature and well organized with pleasant personality and have a professional and warm demeanor that facilitates the development of a collaborative relationship with patients and providers. Successful applicants will demonstrate strong initiative, feel comfortable working at a rapid pace in a busy primary care clinic and will be able to make clinical decisions quickly and confidently. Successful applicants will feel comfortable maintaining availability and flexibility to assess and treat patients presenting with behavioral health needs who are seeing any of several medical providers in the clinic. Appropriate candidates will have a knowledge of psychodynamics, human relations, crisis intervention, psychopathology, and group dynamics appropriate for their level of education and experience. Ideal applicants will have an understanding of Integrated Behavioral Healthcare principles and strong documentation skills. Additionally, successful applicants will demonstrate strong prioritization, organization and problem-solving skills and approach tasks and patient encounters in an energetic and creative manner. Successful applicants must exercise a high degree of initiative, the ability to manage requests from a variety of providers and staff members and be able to collaborate with a diverse group of individuals with various degrees of training/education. Candidates should possess the ability to work in a rural setting utilizing available facilities and resources.  Critical to the role is the willingness to acknowledge and respect the cultural and social background of Health Help’s patients. Successful applicants will also demonstrate flexibility to respond to variances in pace of work, unexpected changes to duties, and high stress situations.


Maintains competency and knowledge of current standards of practice, trends, and developments in related scope of practice through reading and in-services. This is especially important in the areas of insurance coverage and claims, patient flow, scheduling, assuring correct patient information, and handling patient relations.  Obeys appropriate dress code as specified in Employee Handbook.  Provides superior customer service to all patients, external customers (office staff of other offices, hospital, community agencies, etc.), and co-workers.  Serves as an ambassador of the White House Clinics in all conversations and interactions with these parties.
Demonstrates excellent customer service through his/her attitude and actions, consistent with the standards contained in the Mission and Vision and Values of the organization. Maintains clinic standards for a clean and quiet patient environment to maintain a positive patient care experience. Promotes culture of safety for patients and employees through proper identification, reporting, documentation, and prevention.
Maintains compliance with organization’s policies, as well as established practices, protocols, and procedures of the position, department, and applicable professional standards.  Complies with organizational and regulatory policies for handling confidential patient information. Adheres to professional standards, clinic policies and procedures (OSHA, HIPAA, Standing Orders, etc.), federal, state, and local requirements, Health Resources and Services Administration (HRSA), U.S. Department of Health and Human Services (HHS), Office of the Inspector General (OIG), etc. Cooperates fully with all aspects of Corporate Compliance Plan and Standards of Conduct. Participates in all safety programs which may include assignment to an emergency response team.
All Behavioral Health Providers are directly accountable to the Director of Behavioral Health Services.
Certified Social Workers (CSW) in the Behavioral Health Provider role, pursuant to 201 KAR 23:070, are required to remain under the supervision of an LCSW with a Board of Social Work approved Supervision Contract until independently licensed. While the CSW reports to the Director of Behavioral Health Services for employment functions, the clinical supervisor of record maintains responsibility for oversight of the clinical work of the CSW. The CSW is responsible for maintaining appropriate CEU and license requirements.


Requires a variety of physically and mentally demanding activities.  Requires full use of eyes, ears, and hands.  Requires sitting, walking, some bending, stooping, stretching and lifting (generally not in excess of 75 pounds at a time).  Requires ability to prepare written reports and use telephone and computer equipment. Requires ability to move freely and quickly throughout the clinic in order to offer access to primary care patients.
A position exempt from wage and hour laws requires hours in excess of 40 per week.  Work is performed in health care environments including medical office, with potential exposure to communicable diseases.  Possible exposure to aggressive or violent patients.  Requires ability to work under stressful conditions, often for long periods of time.
Evaluated annually by the Director of Behavioral Health Services with input from Supervisor of Record and other staff as appropriate.  Degree of compliance with this Job Description and criteria noted in the employment agreement will be a major aspect of the evaluation.
